Top-opening form holder with storage compartment features an aluminum cover clip that holds approximately 1-1/2" of forms. Antimicrobial additive effectively reduces the formation of bacteria and fungi on the product. Storage compartment holds up to 30 completed forms. Form holder offers a 3/8" storage capacity. Patented clip system provides a high tension clip with license/card holder and a self-locking latch that keeps the compartment securely fastened. Writing plate provides a firm surface for clear copies while protecting forms under the clip. Ideal for healthcare workers, law enforcement and mobile professionals. Form holder is made of aluminum with 60 percent post-consumer recycled material.
